Self-Assessment Tool for Organizational Health Literacy Development (SA-OHL-Thai) The SA-OHL-Thai is a specialized instrument developed in 2018 to transition health literacy (HL) from a purely personal responsibility to an organizational and systemic mandate . It serves as a guiding tool for executives to transform their entities into Health Literacy Organizations (HLO), ensuring that services are accessible and information is easy to understand for all citizens . 1. Background and Design Concept The tool is rooted in the belief that organizations must empower individuals by simplifying complex health systems . It draws inspiration from international standards such as the 10 Attributes of Health Literate Healthcare Organizations (USA) and the V-HLO (Europe), but is specifically adapted for the Thai context using the Thailand Quality Award (TQA) framework . Instead of a traditional strategic focus, this model prioritizes effective communication as a core driver for change . 2. Framework Structure and Scoring The assessment consists of 7 categories, 20 items, and 62 indicators (comprising 58 process indicators and 4 outcome indicators) . Organizations use a 4-level rating scale to measure success, ranging from Level 1 (0-25%) to Level 4 (76-100%) . This evaluation is facilitated through the "HLO Network" application, which generates Radar Charts to help management visualize strengths and identify Opportunities for Improvement (OFI) . 3. The Seven Core Categories Leadership and Governance: Executives must drive an OHL culture, setting clear goals and integrating health literacy into every aspect of the organization’s quality system . Communication: Focuses on both interpersonal and media communication. Design guidelines emphasize font size, visual structure, and the use of plain language while avoiding technical jargon . Customer and Service Recipient Focus: Involves service users in the design and testing of materials. It promotes empowerment through self-help groups and provides education for both patients and their caregivers . Measurement, Analysis, and Knowledge Management: Utilizes real-time data for decision-making and innovation, while sharing successful HLO practices with the wider public . Workforce Development: Ensures staff are trained in patient-centered communication techniques, such as "Teach-back" and "Chunk-and-check," and monitors the health and safety of the employees themselves . HL Process Development: Enhances navigation through clear signage and digital searchability . Crucially, it defines communication errors as "Adverse Events" that require systematic risk management and process improvement . Results: Measures success across three dimensions: Access and Cost, Health Behaviors/Outcomes, and Social Impact (Participation, Equity, and Sustainability) . 4. Continuous Improvement The tool is intended to be a powerful communication instrument rather than a one-time test . Organizations are encouraged to perform self-assessments every 3 to 6 months to monitor progress and systematically close the "Gaps" in their service delivery
