Liquidity Crisis to sustaiable Health System: Transforming From a "Treatment Focus" to PHC and SDG

The Looming Sustainability Crisis The Thai healthcare system, once ranked among the top 10 globally, is currently facing a severe financial sustainability crisis . By 2026, public hospitals faced a massive liquidity shortage, with debts to pharmaceutical companies exceeding 60 billion THB . This crisis is driven by an aging society and the rise of Non-Communicable Diseases (NCDs), which require long-term, expensive care . Furthermore, policy shifts—such as the removal of "co-payments" and the "30 Baht everywhere" initiative—have weakened the Gatekeeper system, leading patients to bypass primary care units for more expensive tertiary hospitals . Economic and Global Pressures Thailand’s health system is under immense pressure from a "Supply Shock" caused by global conflicts, leading to high inflation and low GDP growth . While the national GDP growth is projected at only 2%, medical inflation has soared to 10.8% . This economic disparity, combined with the aftermath of COVID-19, has caused Thailand's Life Expectancy (LE) to regress by 10 years, dropping to 74.3 years in 2021—a level last seen in 2011 . Health Literacy (HL) as a Strategic Lever To overcome these challenges, the sources emphasize a shift from "Fixing" (curative care) to "Building" (prevention and promotion), which offers a much higher Return on Investment (ROI) . Health Literacy (HL) is a critical tool in this transition: Levels of HL: Moving beyond Functional HL (basic reading) to Interactive and Critical HL is essential to spark social movements and advocate for policies that create healthy environments . Impact: Higher HL is directly linked to lower total health expenditures, better health outcomes, and increased equity and empowerment within the population . The Foundation: Primary Health Care (PHC) and UHC The roadmap to achieving the 2030 Sustainable Development Goals (SDGs), specifically SDG 3 (Good Health and Well-being), relies on the synergy between Universal Health Coverage (UHC) and Primary Health Care (PHC) . PHC is described as the "key" to sustainable UHC, requiring three pillars: Integrated Health Services: Comprehensive care including primary and community-based services . Multisectoral Policy: Addressing the broader determinants of health across different government sectors . Community Empowerment: Engaging individuals and families to take charge of their own health . The Strategic Shift for Resilience Thailand must adopt the WHO Framework for Resilience (2022), which focuses on Essential Public Health Functions (EPHFs) and a "Health in All Policies" approach . This involves transitioning from a hospital-centric system to one that is "for the people and by the people," ensuring that current health policies do not create a "disaster" for future generations
